Boarding Form

Standard Package. The Standard Package is included in your boarding fee.

• One on one check-in time with you and your pet
• Food and water intake and elimination monitored
• Bedding provided (towel or blanket)
• Dogs have the use of two runs
• Cats have the use of a cat-condo with perch

TLC Package (Additional daily charge will apply.) Standard Package PLUS

• A photo of your pet text to you each day
• A written report detailing your pet’s stay
• Premium bedding provided (memory foam bed with sherpa cover for dogs; luxury
  bed for cats)
• Bedtime Snack (homemade biscuits for dogs, feline treats for cats)
• Daily one-on-one cuddle or play time with staff
• Cats have the use of a cat-condo with perch

Medications Required

If your need to change your pick-up date, please provide 24 hr notice to allow sufficient time to bathe your dog.

What should stay home? Food bowls and bedding! We provide clean stainless steel bowls and bedding. All bedding is washed regularly and cannot be easily kept track of.

To protect the health of all pets boarding with us, any pet that is diagnosed with intestinal parasites or fleas will be treated at the owner’s expense. Al dogs must be up to date on Distemper, Rabies, Bordetella, and Influenza vaccines. All cats must be up to date on Rabies and Distemper. All pets must have had a fecal test submitted within the last 6 months.

The clinic and staff will not be held liable for problems that develop provided reasonable care and precautions are followed. Every effort will be made to contact you or your designated emergency contact if any problem develops. I understand that any problem that develops with my pet will be treated as deemed best by the staff veterinarians and I assume full responsibility for the treatment expense involved.
